The Home and Community Care (HACC) Program is a joint Commonwealth, State and Territory funded initiative which began in 1985. The HACC Program funds basic support services to some older people, people with disabilities and their carers to assist them to continue living independently at home. This includes:
- Support to participate in social activity in a group or one-on-one
- Assistance with everyday household tasks
- Assistance to enhance nutrition, function, strength, independence and safety
- Assistance to support your independence in your personal care activities such as showering and dressing
- Assistance to keep up with essential activities such as shopping, banking and maintaining social contacts
Wellness Approach
SHINE has embraced the Wellness Approach to Community Homecare by adopting the following essential principles:
- Wellness Approach focuses on building capacities by looking at a person’s abilities, rather than their limitations
- Aims to provide an opportunity to regain confidence and skills whilst supported to do so
- Recognises the value and importance of continued connections with the community
